About Client
NewWave is a US based telecom and healthcare contracting company. Primarily focused on building healthcare systems at scale, they are the backbone that provides millions of people healthcare insurance.
The Problem
Patient health is impacted by various elements and these elements have been defined by SDOH. Different communities in the US have CBOs that attend to some of these social elements that impact patient health. The process of a doctor referring a patient to a CBO typically is done manually. The Patient fills out a hardcopy questionnaire, and the hospital admin/doctor analyzes and determines the kind of aid the patient will need. They source for a CBO and send emails to these CBOs hoping to get one who responds with the ability to provide aid to the patient.
Our Solution
Saffron leverages Social Determinants of Health (SDOH) to build solutions for the health community. Saffron includes the Pulse Application (Practitioner/ Doctor/ Provider) which enables patient assessment and subsequent closed-loop referrals to community-based organizations. Clarity Application (Patient) which enables Patients to self-report in response to surveys, Questionnaires, etc. Magnet Application (Community-Based Organizations (CBO)) which enables CBO users to accept and manage Incoming Referrals.
